Documents Requested by the First Steps Study Committee

  1. The original letter and supporting documents sent to the Internal Revenue Service (IRS) requesting 501 (c) (3) status for the Office of First Steps.

  2. Please complete the attached form regarding local partnerships filing Form 990 with the IRS.

  3. An audited financial report of the Office of First Steps for each of the past three years.

  4. For each of the past three years (including the most recent information available) provide a list indicating which partnerships submitted an audited financial report to the Office of First Steps.

  5. For each of the past three years (including the most recent information available) provide the audited budgets for each county partnership.

  6. Provide a list of each Office of First Steps employee by title, program, and employment status.

  7. Provide an updated and complete list of First Steps board members, the date of their appointment, by whom they were appointed. Please include the date of original appointment and current appointment, and/or total number of years serving?

  8. Provide a budget for each program run by the Office of First Steps.

  9. Provide a copy of the reports required by Act 286, provisos 1.74 and 1.85.

  10. Indicate data being collected from CDEPP providers, how the data is obtained, and whether it is compatible with SLICE.

  11. Provide a list of technical assistance visits conducted by the Office of First Steps to local partnerships over the last year, who conducted those visits, and the purpose of the visits (CDEPP, fundraising, local board development, etc). — (Summary) (Data)
